Insurance Policy

Our goal is to make our services affordable to our patient

Insurance:
We participate with Horizon Blue Cross/Blue Shield Traditional and Delta Dental Premier insurance plans. Please call our office to make an appointment and to ask any questions you may have.

If you have dental insurance we will be happy to process your claims for you.  Please keep in mind that you will have co-payments for almost all of your treatment. Co-payments are expected when the services are completed. In some instances full payment will be requested when services are completed.
